Weekly Wellness Concierge Checks

This service helps keep clients healthy and out of the hospital. Includes:
Weekly 1-hour home visit (or 90 min for 2 clients)
BP, oxygen, temperature, pulse checks
Heart, lung & circulation assessments
Medication checks and planning
Home safety walk-through
Review of test results & doctor updates
Appointment scheduling (as needed)
Education on health conditions
